Title: Client Intake Coordinator
Role Summary
The Client Intake Coordinator you will be the welcoming face of our organisation, supporting prospective and new NDIS participants through every step of their onboarding journey. You will meet with participants, families, support coordinators, and key stakeholders to understand individual goals, provide information about our services, and help design supports that empower people to live their best lives and achieve greater independence.
Working collaboratively with our team, you'll coordinate a smooth and positive intake experience from initial enquiry through to service commencement, ensuring every participant feels informed, valued, and supported from day one. You will also represent the organisation at community and networking events, building relationships with referral partners, promoting our services, and creating opportunities to connect with prospective participants and stakeholders.
Responsibilities and Duties
- Act as the primary point of contact for prospective and new NDIS participants, providing a professional, responsive, and welcoming experience.
- Build strong relationships with participants, families, carers, support coordinators, and other key stakeholders throughout the intake and onboarding process.
- Meet with prospective participants to gain a thorough understanding of their goals, circumstances, support needs, and aspirations.
- Coordinate the end-to-end onboarding process, ensuring a smooth, timely, and positive transition into services.
- Ensure all intake documentation, referrals, service agreements, and participant records are completed accurately and in accordance with organisational and NDIS requirements.
- Work collaboratively with internal teams to coordinate supports and ensure participants receive the right services from commencement.
- Maintain accurate records and monitor referral, intake, and onboarding outcomes using organisational systems.
- Promote the organisation's services within the community by assisting with marketing initiatives and community engagement activities.
- Represent the organisation at networking events, expos, and community forums to build relationships with referral partners and increase awareness of our services.
- Identify and develop new referral pathways and opportunities to grow the organisation's participant network while maintaining strong relationships with existing stakeholders.
This role is being hired on a permanent full time basis and is located at our central office in West Perth.
Skills, Attributes and Experience
Our ideal candidate will have significant previous experience working within the Disability sector and the NDIS framework, including a sound knowledge of NDIS funding and plan structures. They will have excellent analytical, problem solving and conceptual skills, and use these to effectively promote services and assist in gaining new business. Our ideal candidate will have a person-centered, individualised approach to their work and demonstrated proficiencies with relationship building, negotiating and advocating. They will also have the ability to translate their work into succinct, accurate reporting that can be presented to various internal stakeholders. Most importantly, our ideal candidate will have tremendous passion for their work, and for playing a role in the ongoing growth and success of their organisation.
Key Requirements include:
- Previous experience working within the NDIS is essential
- Possess or willing to obtain an NDIS worker screening check
