Role Summary

Mental Health Community Worker are experienced community workers whose roles have an emphasis on working with clients with psychosocial support requirements. The role works within our Mainstream Capacity Building (MCB) team and aims to reduce the readmission cycle to hospital for clients, as well as focusing on barrier minimisation strategies for people living with Disability.

Responsibilities and Duties

  • Assisting clients to live independently and to access supports and services
  • Developing support plans based on the needs of individual clients
  • Monitoring the progress and providing reporting on support plans
  • Liaising with the families and representatives of clients
  • Seeking feedback and ensuring this is used to improve services
  • Networking and connection to services

Skills, Attributes and Experience

Our ideal candidate will have previous experience working in the mental health sector or with clients requiring significant psychosocial support or have recently completed qualifications in a mental health field. They will be a proactive, positive and communicative person who prides themselves on providing quality support services to people who require them. They will have a strong ability to build meaningful, lasting relationships on a foundation of trust and respect. They will be a proficient problem solver who has a confident attitude towards difficult situations and works collaboratively with people to achieve favourable outcomes and results.    

 

As well as the above, the Key Requirements include:

 

  • Applications must possess a Cert IV or higher qualification in a relevant field
  • Possess or be willing to obtain a NDIS Worker Screening Check
  • Hold a valid Driver’s Licence and a reliable vehicle
  • Previous experience working within mental health is highly desirable
