Title: Support Coordinator
Role Summary
As a Support Coordinator, you’ll play a vital role in helping participants understand and implement the supports funded in their NDIS plans. You’ll connect clients with the right services and ensure they have the guidance needed to make the most of their supports. Working closely with clients, their families, and representatives, you’ll empower participants to build the skills and confidence to manage their NDIS supports independently over time.
Responsibilities and Duties
The Support Coordinator role involves supporting clients to understand and implement the funded supports within their NDIS plans, while connecting them with appropriate services to achieve their goals.
Key Duties Include:
- Accepting and following up on referrals for NDIS Support Coordination
- Working collaboratively with clients and their families or representatives to build their capacity to manage supports within their NDIS plan
- Operating effectively within the NDIS funding framework and terms of business
- Identifying and responding appropriately when a client’s needs change or escalate
- Collaborating and liaising with internal teams and external stakeholders to ensure coordinated and effective support delivery
This role is being hired on a permanent full-time or part-time basis, tell us what you're looking for! The role will support clients across Rockingham, Cockburn, Fremantle, Wilson and surrounding suburbs.
Skills, Attributes and Experience
Our ideal candidates will have experience in case management or support coordination within the disability sector and a solid understanding of the NDIS framework and its terms of business. Tertiary qualifications in a relevant humanities field are desirable as well, however, more important than formal qualifications are your ability to practice kindness and empathy, your strong communication skills, and your capacity to work autonomously while remaining connected to a supportive but physically dispersed team.
Key Criteria Includes:
- Previous experience working in a Support Coordination role within the NDIS framework
- Strong interpersonal skills with the ability to build and maintain lasting professional relationships
- Creativity and resourcefulness when solving problems and navigating complex situations
- A high level of self-motivation and a genuine drive to deliver positive outcomes for your clients
