Role Summary

The role of Support Coordinator is centered around supporting clients to understand and implement the funded supports in their NDIS plans, as well as linking them to appropriate services. You will work in collaboration with clients and their families/representatives to build up their capacity to manage the supports within their NDIS plan. In doing so, you are tasked with going above and beyond to support your clients in reaching their goals and pursuing their dreams.

Responsibilities and Duties

The Support Coordinator role involves supporting clients to understand and implement the funded supports in their NDIS plans and linking them to appropriate services.

Key Duties Include:

  • Accepting and following up referrals for NDIS Support Coordination
  • Working in collaboration with clients and their families/representatives to build their capacity to manage the supports within their NDIS plan
  • Working effectively within the NDIS funding framework and terms of business
  • Directing appropriate responses in circumstances where a clients requirements have changed or escalated
  • Collaborating and liaising with internal and external stakeholders as required

This role is being hired on a full time permanent basis, and works out of our tight-knit Rockingham office!

Skills, Attributes and Experience

Our ideal candidates will have previous experience in case management or support coordination within the Disability sector and be familiar with working within the NDIS framework and terms of business. Tertiary qualifications in a relevant humanities field are desirable. However, your capacity to practice kindness and empathy, your communication skills and your ability to work autonomously within a closely connected but physically distanced team will be more important than qualifications. 

Key Criteria Includes:

  • Previous experience working within a support coordination capacity within the NDIS framework
  • The people skills to grow and maintain lasting professional relationships
  • The ability to bring ingenuity and creativity to solving problems
  • The drive to self-motivate and push yourself to deliver for your clients
