Title: Community Services Coordinator
Role Summary
We are seeking a passionate Community Services Coordinator to ensure the delivery of high-quality, person-centered services that meet the needs of our clients. In this role, you will engage regularly with clients, their families, and other stakeholders, building meaningful and lasting relationships. You will oversee staffing levels, manage the performance of support workers, and ensure services are delivered effectively within budget. Responsibilities and Duties
- Work closely with clients to ensure their funds and services are used effectively and meet their needs
- Ensure support plans are followed and maintain high-quality progress notes
- Manage, support, and develop a team of support workers
- Ensure services comply with relevant standards and regulations, and manage risks effectively
This role is being hired on a permanent full-time basis and is located out of our central office in Belmont, with day travel to Bunbury required once or twice a week; a work pool car is provided for the travel.
Skills, Attributes and Experience
- Relevant coordination experience within the NDIS or Aged Care framework is essential
- Relevant qualifications or certifications within the industry is highly desirable
- Excellent documentation and reporting skills
- Strong interpersonal skills with the ability to communicate effectively with a range of stakeholders
- Must possess or be willing to obtain an NDIS Worker Screening Check and a Working with Children Check
- Obtain a valid driver's licence with access to a reliable vehicle
