Role Summary

The Lifestyle Coordinator’s primary goal is to develop and oversee a lifestyle program that reflects client preferences and enables individuals to maximise their abilities with an appreciation of the physical, social, cultural an d religious background each person possesses. The role also helps to mentor and lead in the delivery of programs that are a focal point of social activity & emotional connection for residents. Our Lifestyle Coordinators build rapport with our residents, liaise with relevant health professionals and family members as required.

Responsibilities and Duties

  • Establish a positive, supportive and encouraging line of communication with each resident and their next of kin, in order to produce activity assessments and care plans that are reflective of the resident's interests & abilities
  • Develop, implement and evaluate individual, social and leisure care plans. Mentor the team in how best to implement these for each client
  • Identify a variety of entertainment options (internal & external within an agreed budget) and incorporate these into a monthly program. Promote and engage with the residents around this program
  • Develop positive relationships with support staff, nurses and volunteers at the facility in order to ensure a comprehensive, compassionate and holistic approach to the social, emotional & cultural wellbeing of each client
  • Ensuring that lifestyle coordination functions with a clear awareness of organisational policies and with a clear approach to any potential risk or hazards, addressing issues immediately

Skills, Attributes and Experience

  • Experience supporting residents with dementia is highly desirable 
  • Ability to relate to elderly clients with empathy whilst working effectively with other health professionals and service providers
  • Demonstrated success working in an aged care setting including experience in assessment and equipment prescription
  • Experience in the development and delivery of therapy based programs
  • Demonstrates a commitment to being a productive and positive team member 
  • Excellent interpersonal, communication and time management skills
  • Knowledge of community based programs and a flexible approach to working within these
